　頭部外傷急性期における早期予後判定に有用である因子について，年齢，入院時神経学的重症度，凝固線溶系異常，画像所見など多角的に検討を加えた。対象は受傷後6時間以内に入院した頭部外傷51例（男性34，女性17）に対して，凝固線溶系特に入院時fibrin degradation product（FDP）とCT分類，神経学的重症度などについて検討を加えた。入院時FDP＞40μg／mlを示した症例のうち44％が重症頭部外傷例であった。FDP上昇と硬膜下血腫および挫傷性脳出血例とは相関がみられた．FDPと転帰の関係では，FDP正常群では死亡例はみられなかったが，FDP著増群ほど死亡率が高値であった。入院時血清FDPの上昇は脳挫傷の量を反映していると考えられ，血清FDP値は頭部外傷の早期予後判定に有用であると考えられた。

To identify early prognostic value of brain injury, a comparison was made between computerized tomography (CT) findings, coagulation abnormal-ities, and clinical features in 51 patients with closed head injury. The patients were divided into three groups according to their plasma level of fibrin-fibrinogen degradation product (FDP) : normal group (FDP 10μg/ml or less) in 20 patients ; mode-rately abnormal group (FDP 10-40μg/ml) in 15 patients ; and highly abnormal group (FDP 40/μg/ ml or more) in 16 patients. Cases with a fatal clinical course were mostly associated with very high FDP level.
