　転倒時に雑草の茎が経鼻的に頭蓋底を穿通して脳内出血を起こした1例を報告した。来院時，外来医が鼻腔に刺さった茎を抜去すると，鼻出血が起こり，意識も急速に障害されてきた。緊急に開頭し，右前頭葉内血腫を除去し，穿通部の形成術を行った。穿通性脳挫傷の受傷機転や診断，治療上の問題点を考察し，頭蓋内の詳細な放射線学的検索なしに穿刺物を抜去することは非常に危険であると強調した。

A case of penetrating head injury caused by weed was reported. A 69-year-old man fell from a bicycle and was stuck by the hard stalk of weed through the right nasal cavity. On admission the patient was fully alert and with no neurological deficits. The weed was pulled out at an out-patient department and then he became semicomatose and hemiplegic on the left side together with an occur-rence of nasal bleeding. Subsequent computed tomographic (CT) scan showed an intracerebral hematoma in the right frontal lobe.
