　特発性副甲状腺機能低下症（IHP）の原因は種々あるものの，低カルシウム（Ca）血症のために多彩な症状がみられ，特に中枢神経組織（CNS）の石灰沈着を呈し，そのため頭部X線や脳CT・MRIで発見されることが多い。当症例は2-3年前から話すことに困難さを感じ始め，特に大声が出し難く平成5年8月，某院でのMRI検査で頭部の石灰化を指摘され精査のため当科に紹介された。低Ca血症の他，副甲状腺ホルモン，1.25（OH）₂Dが著明に低下し，特発性副甲状腺機能低下症と診断し，血清Ca，P，creatinineを指標として1α（OH）D₂2μg／dayを投与した。手指けいれん様振戦は消失したがパーキンソニズムと病態が異なる発語障害は改善せず約1年後のCT所見に変化はみられなかった。本例の如く成人で症状が出現した場合，CNSの石灰沈着を除去することは困難であるため，早期に発見し，ラット低Ca食実験の成績で示唆されたように血中Caを是正すればCNSの症状発現および進行を予防し得る可能性が考えられた。

A case of idiopathic hypoparathyroidism (IHP) is discussed with reference to a study of rats maintained on various low-calcium diets. A 45-year-old man complained of difficulty speaking, especially an inability to speak loudly and of fine finger tremors over the past 2 or 3 years. His brain MRI showed marked calcification in the bilateral basal ganglia and cerebellum and his in sera findings showed low calcium, extremely low levels in all kinds of examined parathyroid hormones and 1,25 (OH)₂D, thus we diagnosed idiopathic hypopara-thyroidism.
