The pulsating exophthalmos due to intra-orbital encephalocele or meningocele associating with orbital bone defect is rare phenomenon.

Recently, we have experienced a case of 40 years old female showing uniteral exophthalmos. On the symptom, it was characteristic that this exophtha-lmos subsides and rather sinks after removal of cerebrospinal fluid by lumbar puncture and P. E. G. procedure. An acurate diagnosis was established by means of various X-ray examinations.

On the operation we found the meningocele located on the right temporal tip and accorded with the retro-orbital bone defect, extending to the orbital roof, lateral side of small wing and cerebral side of large wing of sphenoidal bone. The re-moval of the meningocele and plastic procedure with tetron mesh for the bone defect brought to comlete disappearance of the exophthalmos.

We reported a case and concidered in reference to this problem on the literature.
