大脳萎縮を示した脊髄小脳変性症の2症例

TWO AUTOPSY CASES OF THE SPINO-CEREBELLAR DEGENERATION WITH THE COMBINATION OF THE CEREBRAL ATROPHY

緒言

　脊髄小脳変性症に関しては本邦においてもその臨床例についての研究は多い。しかしながら剖検例についての報告は意外に少く，ことに遺伝性，家族性に発現した例に関して然りである。遺伝性—家族性症例中オリーブ核・橋・小脳変性の形をとるMenzel型の報告も少いが，小脳・オリーブ核性の形を示すHolmes型の報告例は極めて稀である。

　一方，脊髄小脳変性症に精神症状の合併することはMarie, Londo以来知らられていたことであり，ことに長期経過例については精神症状の合併はほとんど必発とされ，高度の痴呆を記載する者も多い。またこのような症例に大脳病変を認めた報告も見られる。

Abstract

Clinical and neuropathological findings of two cases belong to the spino-cerebellar degeneration have discussed. Both cases have the familial manifestation. One of these belongs to the olive-ponto-cerebellar atrophy of the Menzel type, and the other to the cerebello-olivary degeneration of the Holmes type. Both cases combine the cerebral atrophy. About the combination of the spino-cerebellar degeneration with the cerebral atrophy have discussed clinico-pathologically.

Also, Case I shows the changes of the posterior column of the spinal cord, substantia nigra, and Case 2 shows the changes of the spinal cord, the striatum as the so-called accessory localisation.
