A case of spino-cerebellar heredo-ataxia with transient delirious state and frequent attacks of grand mal seizure is presented. The patient is thirty-five years old man, whose parents are cousin. In history of present illness, the patient had noticed the disturbance of gait when he was 24 years old. He was admitted to our hospital with ataxic walk, scanning speech, intention tremor, adiadochokinesis and slight dementia at age of 32. The patient complicated by grand mal convulsion since about two years. Later, grand mal convulsion, spino-cerebellar symptoms and dementia had developed gradually, the patient also revealed anxious state and child-like attitude. In transient delirious state, he had visual hallucinatory paranoid state occasion-ally.

On the laboratory examination, nothing abnor-mality was observed except B. S. P. for liver function test, electrocardiogram and electroencephalogram.Electroencephalogram showed flat waves in general and poor a waves in all leads, but no anymmetrical, spike discharge and focal sign. Electrocardiogram also showed non conducted atrial premature beats and nodal beat which were suggested myocardial damage. B. S. P. value were 20. 8 per cent to 30 min. and 14.4 per cent to 45 min.
