A case of arteriovenous fistula between the external carotid artery and the transverse sinus with symptoms of intracranial hypertension, resembling brain tumor is reported.

A 43-year-old man had suffered from headache and visual disturbance for about a half year and he fell suddenly into unconsciousness following lumbar punc-ture.

Papiloedema existed in both eyes. Routine radiograms of the skull disclosed oval translucent area in the occipital bone. Left carotid angiograms revealed the markedly dilated and tortuous occipital artery. The transverse, straight, sigmoid and inferior sagittal sinus were visualized in arterial phase with granulated sha-dows in the occipital bone. Ligature of the external carotid artery performed successfully.

This case may be presumed to be a kind of arte-riovenous malformation in occipital bone.
