外頸動脈・海綿静脈洞瘻—自験例並びに文献6例について

EXTERNAL CAROTID-CAVERNMTS SINUS FISTULA: A CASE REPORT AND 6 CASES IN LITERATURE

はじめに

　脳動脈瘤，脳動静脈奇型，脳動脈閉塞症など脳血管障害の診断に脳血管撮影は不可欠のものであるが，近来この普及により種々の脳血管障害が報告されろようになつた。

　我々は最近，眼球突出，結膜充血，頭蓋内雑音等をきたし，あたかも内頸動脈海綿静脈洞瘻を思わせたが，脳血管写の結果外頸動脈海綿静脈洞瘻であることが判明した例を経験した。我々が調べた範囲では文献上いままでに外国で5例の^{1）6）7）8）9）}，本邦では1例^5）の報告がある。

A Case of the spontaneous external carotid-caver-nous sinus fistula was reported.

A 42 year old woman had suffered from the ex-ophthalmos, chcmosis, and Mtracranial bruit. Left external angiogram revealed the fistulous communi-cation between branches of the external carotid artery and inferior pctrous sinus, cavernous sinus, and deep cerebral veins. Embolization and ligature of the left external carotid artery was performed, but unsuccessfully.

By the review of 6 cases of the external carotid-cavernous sinus fistula ever reported in the liter-ature, we found most of them to occure spontane-ously, and the difficulty of improving was proved by the operation, the ligature of the external carotid artery.
