外頸動脈—頭蓋内静脈洞瘻—自験例および文献9例の総括的考察

EXTERNAL CAROTID-INTRACRANIAL SINUS FISTULA:A Case Report, and 9 Cases of the Fistulae in Literature

I．はじめに

　近来，脳血管撮影の普及につれ，種々の脳血管異常の存在がしだいに明らかにされつつある。これらの一つとして脳動静脈短絡があり，脳動静脈奇形（A-V malfor—mation）および内頸動脈—海綿静脈洞瘻（carotid-cava—nous fistula）は，その代表的なものとして従来から一般に知られていた。しかし，外頸動脈—頭蓋内静脈洞瘻については近年に到るまでその報告がなく，1951年Fincher^1）が最初の報告を行なつて以来その存在に注意が払われるようになり，現在まで文献上9例の報告^1）8）をみるに過ぎない。

　われわれは最近，後頭動派—横静脈洞瘻の1例を経験し，自験例および文献上の外頸動脈—頭蓋内静脈洞瘻9例について検討を加えたところ，これらは，その成因，症状，診断，治療などの点で前述の脳動静脈奇形，内頸動脈—海綿静脈洞瘻とは異なつており，外頸動脈—頭蓋内静脈洞瘻として一括されるべき性質のものと考えたので，われわれのまとめ得た結果について報告する。

A case of the occipital artery-sinus transversus fistula was reported, with 9 cases of the external carotid-intracranial sinus fistula which have so far reported in the literature. We summerized and dis-cussed causes, clinical signs, and operation of the fistula on the bases of these 10 cases of the external carotid-intracranial sinus fistulae. The fistula was caused by trauma in 7 cases, by lumbar puncture in 2 cases, and by obscure cause in 1 case.
