Cerebral angiography represent one of the most important diagnostic aids found in the armamenta-rium of the clinician endeavoring to explore obscure intracranial hematoma. However, it is often difficult to determine whether it is extradural or subdural in origin.

Recently we experienced a case diagnosed as an extradural hematoma preoperatively by the exter-nal carotid angiography which showed extravasation of contrastmedium from the middle meningeal artery and verified by the craniotomy.

There have reported the extravasation of the contrastmedium from the middle meningeal artery through the common carotid or external carotid angiography by many authors, in which most of them were taken with in 6 hrs. after the trauma. In the past, the reported longest time intervals be-tween the trauma and angiography was 32 hrs.

However, we detected an interesting case repre-senting the extravasation of the contrastmedium in the external carotid angiogram which was taken 100 hrs. after the trauma.
