新ニコチン酸誘導体Meso-inositol-hexanicotinateの精神神経科領域への応用

CLINICAL EXPERIENCES WITH A NEW NICOTINIC ACID DERIVATE-MESO-INOSITOL-HEXANICOTINATE IN THE NEUROPSYCHIATRIC DISEASES

I．はじめに

　ビタミンB群のひとつであるニコチン酸は強力な血管拡張作用をもち，かつ脂質代謝異常を補正する作用のあることが明らかにされている。精神神経科領域で最近15年間に症状精神病の原因のひとつとしてビタミン欠乏があげられ（Spillane '47^1）），このうちニコチン酸をはじめB群欠乏による障害にいわゆる栄養性神経障害nutritioned neuropathy （Cruikshank '60^2））なる名が冠されるようになつた。この範疇にはペラグラ精神病やWernicke'仮性脳炎をはじめニコチン酸欠乏性脳症などが含まれている。しかしこのような重篤な障害もその発病の初期にはかなり長期間先行する神経症様症状が認められている。それは抑うつ，焦躁感，全身倦怠，記憶障害，思考の混乱などで同時に食欲不振，悪心，不眠，便秘，運動時の呼吸困難などの自律神経症状を呈している。

　われわれは従来から本領域の各種疾患のうち自律神経症状を呈するものにニコチン酸をはじめとするB群を投与し，その効果によつてlatentの栄養性神経障害の治療に努めてきた（中沢ら'61^12） '62^13））。このような試みはすでに1910年Syden—strideらにより脳動脈硬化性意識障害の治療として，1943年Hardwick^3），1952年Leigh^4），1955年Hersovらによるペラグラ精神病の治療へ，さらに1952年Lehmannによるうつ病の，また1962年Osmond^5）らによる精神分裂病への治療（大量療法）などに用いられている。

The authors studied the effects of a new nicotinic acid delivate (meso-inositol-hexa-nicotinate) on 36 neuropsychiatric patients including the cerebrovascular diseases, autono-mic diseases neuritis posttraumatic neurosis, poisoners and spinomuscular diseases. In all cases, meso-inositol-hexanicotinate was admi-nistrated in addition to other drugs (thiamine, miner tranquilizer) which has been continously given.

Remarkable effects were observed particul-arly in autonomic syndrom (headache, sleep-lessness, nausea, disturbance of consciousness and perception, and blood pressure) and also observed to dicrease the total cholesterol in serum on arteriosclerosis of 11 cases. Among 9 cases with various neurosis, 6 cases were improved psychic conditions (amnesia, hallu-cination and anxiety).

No remarkable side reactions were observed except gastrointestinal disorders such as na-usea and vomiting in a few cases.

The authors accordingly feel that meso-inositol-hexanicotinate is useful enough when supplementarily administered with various tranquilizing agents which have been used effectively.
