痙攣発作を伴つたGroenblad-Strandberg症候群（L'elastorrhexie）の1例

A CASE OF ANGIOID STREAKS OF THE RETINA WITH EPILEPTIC SEIZURES (L'elastorrhexie)

I．はじめに

　網膜色素線条は1889年Doyneが初めて報告し，1892年Knappが命名して以来幾多の報告が発表され，1929年Groenblad u．　Strandbergおよび1931年Marchesani u．　Wirzは網膜色素線条と弾力線維性仮性黄色腫との併発例を報告しその成因は全身の弾力線維の系統的変性にあるとした。以来この併発例はGroenblad-Strandberg症候群として報告されている。さらにその後網膜色素線条とPaget病との併発例（Terry症候群），高血圧性〜心臓血管症との併発例（Tovai—ne's systemic elastorrhexia），円板状黄斑変性との併発例（Coppez et Danis症候群）等が発表され，M．　Appelmans u．　P．　Lebasはこれをひとつの系統づけられたL'elastorrhexieの範？に属するとしている。だが一方ではGroen—blad-Strandberg症候群も拡大解釈されて，McCaugheyはその特徴として弾力線維の広範囲の崩壊変性による1）皮膚，2）眼，3）内臓症状をあげている。

　このように現在では網膜色素線条は全身の弾力線維の系統的変性疾患の部分症状として考えられてきたが，近時本症が神経症状を合併した症例もUrbach Dixon Frederick等により報告されている。われわれは痙攣を主徴候とし，眼底において網膜色素線条を認め，全身の血管系の障害を示した1例を経験したので報告する。

The sixty-nine years old male patient had his first epileptic seizure at the ago of sixty-seven and thereafter he had convulsive sei-zures once a month or so with intellectual deterioration. Various examinations revealed the follwing results:

Vorhof flutter,-flimmern in ECG, swelling of aorta in roentgenogram and positive Rumpel-Leedés phenomenon, continuous and diffuse α rhythms in EEG and typical picture of angi-oid streaks of retina.

It is suggested that the changes of cardio-vascular system were due to the systematic degeneration of elastic tissues and also the neurological and psychological changes might be explained by the same mechanism.
