The case was of a 20 year-old male suffered from organic hyperinsulinism due to an islet cell adenoma of the pancreas, which was hi-stologically ascertained.

1) At first he was diagnbsed as an epilep-tic because of his paroxysmal unconscious-ness, convulsion state and abnormal electro-encephalograms.

2) Careful regard to his signs and estima-tions of blood sugar levels led to the diagno-sis of pancreatogenous hyperinsulinism, thou-gh the clinical symptoms gave an impression of a primary lesion of the central nervous system.

3) The glucose tolerance test after a stan-dard preparatory diet was very important to make a definite diagnosis of pancreatogenous hyperglycaemia.

4) The organic signs of the Rorschach test seemed to be a worthy index to detect an organic disturbance of the central nervous system.
