Patients are reported to have the judgment ability to provide consent, submit in writing about their expectations of the content of treatment if and when their judgment ability happens to be severely degraded. “Psychiatric Advance Directives”(PADs)are known as a way to practice psychiatric care that respects the autonomy of patients at a stage when they lack the judgment ability to provide consent. Currently, PADs have legal implications in more than half of the states in United States of America. The use of PADs in real clinical settings has not been reported in our country. In this study, we created a PADs creation support tool that follows a format based on the opinion of 4 patients. In this study, we present the PADs creation support tool that reflects the opinions of patients themselves and discuss the significance of creating PADs.
