　患者の同意判断能力が欠如している場合でも，自律性を尊重した精神科医療を実践するための方法として，患者本人の同意判断能力が保たれていると判断できる時期に，自身の同意判断能力が著しく低下した際にどのような対応を希望するかについて書面で記載しておく「精神科事前指示(以下，PADs)」が知られている。わが国においてはPADsの実臨床での使用は報告されていない。今回，PADs作成支援ツールを作成し，患者本人の意見を基に書式の修正を行った。本稿では患者本人から得られたPADsに対する意見およびそれらの意見を反映したPADs作成支援ツールを提示し，PADs作成の意義について考察した。

Patients are reported to have the judgment ability to provide consent, submit in writing about their expectations of the content of treatment if and when their judgment ability happens to be severely degraded. “Psychiatric Advance Directives”(PADs)are known as a way to practice psychiatric care that respects the autonomy of patients at a stage when they lack the judgment ability to provide consent. Currently, PADs have legal implications in more than half of the states in United States of America. The use of PADs in real clinical settings has not been reported in our country. In this study, we created a PADs creation support tool that follows a format based on the opinion of 4 patients. In this study, we present the PADs creation support tool that reflects the opinions of patients themselves and discuss the significance of creating PADs.
