PCPS装着時の血管損傷により腹腔内出血を来した2症例の検討

Potentially Lethal Misplacement of Femoral PCPS (percutaneous cardiopulmonary system) Catheters

　開心術後の重症心不全に対しPCPSを装着したところ直後から腹部膨満を起こし死亡した2症例を経験した．剖検の結果，1例は下大静脈に穿孔が認められ，1例は左大腿静脈が外傷により破裂していることが明らかとなった．また，ともに血管損傷部から大量に腹腔内に出血していたことが認められた．PCPS装着時のブラッドアクセスとして2例とも左大腿静脈に脱血管を挿入したが，その際にガイドワイヤーもしくは脱血管により静脈損傷を引き起こした可能性が考えられた．ICUや手術室などにおいて緊急にX線透視なしでPCPSを装着する際は血管損傷の合併症を念頭において慎重に処置を行う必要があると思われた．

PCPS (percutaneous cardiopulmonary system) has been widely used in cases of severe heart failure. This case report describes two cases of lethal misplacement of femoral PCPS catheters after cardiovascular surgery.The two patients, who both suffered LOS (low output syndrome) after cardiovascular surgery, had femoral PCPS catheters inserted, but died from peritoneal hem-orrhage due to misplacement of the catheters. It was found at autopsy that the respective causes of death were perforation of the inferior vena cava and trau-matic rupture of the left femoral vein (due in both cases to poor insertion of the PCPS catheters). In both casesthe left femoral vein had been used for blood inflow. When we use PCPS in emergency situations, it is neces-sary to take great care to avoid a lethal misplacement of femoral catheters.
