Simpson peripheral atherectory was used to treat 37 patients with a total of 111 lesions of the iliac artery (n=46) and femoral arteries (n=65). Initial success was achieved in 107/111 lesions and the success rate was 96%. The stenoses were reduced from 64±20% to 24±13%. Major complications were met with in 2 cases of perforation which required emergent operation and in one case with giant hematoma. Primary atherectomy without balloon angioplasty showed significantly less occurrence of arterial wall cracking or dissection. Pathological findings revealed that media was found in 18 lesions (18%) and adventitia in 7 lesions (7%). Angiographical restenosis was observed in 17% (8/47 lesions) after 7.5 months. Significant risk factors for restenosis were femoral lesions, adjective balloon dilat-ed lesions, and non extension of resection in media or adventitia. Simpson peripheral atherectomy was found to be a useful technique for peripheral vascular inter-vention.
