診断確定後20年間経過観察中である原発性肺高血圧症の1例

A Case of Primary Pulmonary Hypertension with a Stable Clinical Course of 20 Years

　原発性肺高血圧症の予後はきわめて不良であり，稀に10年以上の長期生存例や自然寛解例がみられるものの，ほとんどは症状発現後2〜3年の経過で死亡している．われわれは原発性肺高血圧症の長期生存患者の1症例を経験したので報告する．症例は25歳，男性で，4歳時に原発性肺高血圧症と診断された．診断確定後経時的に血行動態変動を観察するも，肺動脈圧は60／40mmHg前後でほぼ安定を示し，肺動脈楔入圧も10mmHgと正常範囲内に滞っている．約10年間はニフェジピン投与を実施しているが，日常生活もNYHAⅡ度と社会生活が可能である．胸部X線写真，心電図，心臓カテーテル検査などの20年間における推移を文献的考察を加えて検討した．

Primary pulmonary hypertension (PPH) is usually afatal disease with survival of 2-3 years after the onsetof symptoms.However,several patients who survivedmore than 10 years,are reported and regression ofpulmonary hypertension was observed in a few cases.We describe the serial electrocardiographic,radiologi-cal and haemodynamic assessment of a young patientwith a long clinical course.The patient is a 25 yearmale who was diagnosed at the age of 4 as PPH.
