　開心術後患者に対するアムリノンの血行動態に及ぼす影響を，連続心拍出量測定システム（CCOM）を用いて検討した．はじめに，20例に対してCCOMで得られた心係数と，熱希釈法によつて得られた心係数との比較を行い，相関係数が0.72とほぼ良好な相関を示すことを確認した．次に，13例を対象にアムリノン5μg／kg／min持続静注を60分，続いて10μg／kg／min持続静注を60分，投与中止後60分の合計180分間循環動態を連続測定し，アムリノンの影響を検討した．心係数は30分後より有意に増加し120分後に16％変化した．体血管抵抗係数は20分後から有意に低下し120分後に20％の変化を認めた．そしてこれらの変動は，投与前の心係数が2.8未満の群で大きかった．血圧，心拍数は5％以内の変動で有意な変化はなかった．以上より，開心術後症例でのアムリノンの低用量からの持続静注法は，血圧に及ぼす影響を最小限に抑え，血行動態の改善を得る1つの安全な方法と考えられた．

The effects of intravenous infusion of amrinone wereexamined in patients after open heart surgery with acontinuous cardiac output monitoring (CCOM) cath-eter.

In order to examine the accuracy of cardiac outputmeasurement with CCOM,we compared a total of 122paired values of CI obtained by CCOM and conventionalthermodilution method (TD) in 20 patients.Thecoefficient was 0.72 between CCOM and TD.Theseresults suggest that CCOM is useful for the continuousmonitoring of cardiac output.

Hemodynamic effects of amrinone were studied in 13patients.
