　大動脈弁逆流（AR）にみられるstrain型LVHと陰性U波の意義を弁置換術との関連を含めてretrospectiveに検討した．対象は64名の虚血性心疾患を合併しないⅢ度以上のAR患者で，strain型LVHと陰性U波の形状から各々を3群に分類して，心エコー図から得られる指標を用いて比較した．strain型LVHの進行は左室心筋重量の増加を示していた．一方，陰性U波の進行は心室径の拡大，心収縮能の低下を示していた．また，大動脈弁置換術（AVR）の施行との関連をみると，strain型LVHとは一定の関連を示さなかったが，AVRを行った全例に陰性U波が認められ，術後，心室径，収縮能の改善に伴い陰性U波は消失した．以上よりAR患者の心電図にみられる定型的陰性U波の出現は，それだけでAVR施行を決定する指標にはならないとしても，その適応を考慮する一つのサインとしては，有用な所見になりうると考えられた．

Clinical significance of LVH with strain and negative U wave in aortic regurgitation (AR) with special emphasis on decision making concerning surgical inter-vention was retrospectively investigated in 64 patients with Doppler echocardiographically proven AR of severity above grade Ⅲ. Those who suffered from ischemic heart disease were excluded from the present study. All subjects were classified into 3 groups based on either of the 3 types of LVH with strain criteria (no strain, atypical strain, typical strain) or 3 types of negative U wave criteria (on negative U, negative-positive U, symmetric negative U).
