左上大静脈遺残症を伴う洞機能不全症候群に対する経静脈的ペースメーカー植え込み術の1例

Transvenous Pacemaker Implantation of Sick Sinus Syndrome associated with Persistent Left Superior Vena Cava

　症例は70歳，女性で，胸部不快感と徐脈を主訴に当科に紹介された．洞機能不全症候群の診断で，恒久的ペースメーカー植え込み術を施行した．左僥側皮静脈より電極を挿入し，そのとき初めて左上大静脈遺残症に気付いた．そのまま電極を右室まで進め，良好なペーシングが得られた．術後に施行した静脈造影で，右上大静脈の存在と左無名静脈の欠損が確認された．術後の合併症は認められなかった．左上大静脈経由のペースメーカー挿入でも安全なペーシングが得られると思われた．

A 70-year-old woman with a persistent left superior vena cava (PLSVC) underwent pacemaker implanta-tion for sick sinus syndrome. PLSVC was discovered unexpectedly during the insertion of the electrode. The electrode was able to be advanced to the right ventricle through the PLSVC. The electrode tip was placed onto the adequate portion of the right ventricle and satisfac-tory pacing was obtained.
