鏡像型右胸心を合併した洞機能不全症候群に対する経静脈型ペースメーカー植え込み治験

Transvenous Pacemaker Implantation for Sick Sinus Syndrome with Mirror-Image Dextrocardia

　鏡像型右胸心，両側上大静脈，下大静脈欠損・奇静脈接合を伴った洞機能不全症候群の稀な1例を報告した．患者は45歳女性．めまいを主訴として某医を受診．電気生理学的検査中に大静脈系の異常が疑われ，静脈造影にて上記異常が判明した．症状が軽度で，経過観察となっていたが，2年後よりめまいが頻回となり，Holter心電図にて心房細動と最大4秒の心停止を認めたため，ペースメーカー植え込みの適応として当科紹介となった．静脈造影では右上大静脈が冠状静脈洞に灌流していたのに対し，左上大静脈は直接右房に灌流していたので，左側からアプローチによる経静脈型ペースメーカー植え込みを行った．鏡像型右胸心では大静脈系の異常を伴うことが多いとされており，術前の静脈造影は電極の挿入経路を決定するうえで，必須の検査であると思われる．

A case of sick sinus syndrome with mirror-image dextrocardia which was associated with bilateral supe-rior vena cava and an absent inferior vena cava with azygos continuation is reported.

A 45-year-old woman was referred to our hospital with the chief complaints of dizziness and palpitation. The electrocardiogram showed a atrial fibrillation with a 4-second period of asystole. A permanent endocardial bipolar demand pacemaker was inserted through the left superior vena cava.
