　症例は48歳，女性．頭痛，嘔吐，発熱を主訴に当院受診．頸部硬直あり，胸骨右縁第2肋間に拡張期雑音が聴取され，心エコーでは中等度の大動脈弁逆流が認められた．髄液・血液培養いずれからもB群溶連菌が検出され，細菌性髄膜炎および心内膜炎と診断された．ただちにペニシリン系およびセフェム系抗生剤を投与し，炎症所見は改善したが，大動脈弁逆流は増悪し，心不全が進行したため，大動脈弁置換術を施行した．成人のB群溶連菌による感染性心内膜炎は稀であり，本邦ではこれまでに1例しか報告はない．致死率は40％以上とされ，急性・亜急性の経過をとるものが多いとされている．また弁尖の破壊が強く，心不全が進行すると同時に，塞栓症を起こすことも多いとされ，早期手術の必要性が高いと考えられる．

A 48-year-old woman was admitted to our hospital with the chief complaints of fever and headache. Two-dimensional echocardiography revealed the presence of aortic regurgitation and the vegetation on the aortic valve. Blood and liquor cultures were positive for group B streptococcus. With the diagnosis of infective endocarditis complicated with meningitis, the patient was given antibiotic treatment. In spite of medical treatment, congestive heart failure due to aortic regur-gitation became worse, and aortic valve replacement was undertaken on the 49th day after admission.
