We implanted a pacemaker in a 56-year-old woman with sick sinus syndrome who had a persistent left superior vena cava (PLSVC). She was admitted faint-ness. Echocardiogram showed a dilated coronary sinus. We suspected an anomaly of venous return. Venography showed PLSVC with right superior vena cava. Electro-physiological study showed that sinus node recovery time was 5.24 second. An AAI lead was inserted via the right subclavian vein. In view of the lack of symptoms, after the implantation of the pacemaker, we examined 67 cases reported in the past. We discussed which approach should be selected to avoid complications when implanting a pacemaker in cases of PLSVC.
