Denopamineが著効した高齢難治性冠攣縮性狭心症例

Refractory Vasospastic Angina Relieved by Denopamine

　78歳，男性．不安定狭心症にて過去2回の入院歴がある．今回2年ぶりに服薬にもかかわらず狭心症発作が頻発し，次第にNitroglycerineで寛解せず入院となった．緊急冠動脈造影で右Seg．4完全閉塞，左Seg．7 90％狭窄を認めた．左右各々にISDNを冠動脈内注入したところ，狭窄は消失しspasmであることが判明した．各種Ca拮抗薬，硝酸薬，Nicorandilなど5薬併用にても発作は抑えられず，α₁受容体遮断薬のPrazosinも無効であった．ところがβ₁受容体刺激薬であるDenopamineの投与を試みたところ，発作は劇的に消失した．後日，右冠動脈内にNoradrenalineを注入したところ，1μgでは拡張し5μgでは自然発作と全く同一の冠攣縮が誘発された．攣縮の発生にα受容体の関与が示唆されるが，β受容体の刺激がその抑制に働き，難治性冠攣縮性狭心症例では血圧の低下を伴わぬDenopamineの投与を試みるべきと考えられる．

A 78-year-old man was admitted with severe chest pain not relieved by nitroglycerine, and ST elevation appeared in leads Ⅱ, Ⅲ, _aV_F of the ECG. Emergent coronary angiography showed total obstruction in the distal portion of the right coronary artery (Seg. 4), and 90% stenosis in the proximal portion of the left anterior descending branch (Seg. 7). After intracoronary admin-istration of isosobide dinitrate, obstruction and stenosis disappeared and no organic stenosis was seen in coro-nary arteries. It proved to be spasm.
