症例は25歳女性,食事(小麦)摂取後の運動中および運動後に薄麻疹に引き続く失神を2年間に3回繰り返した.心電図,24時間ホルター心電図,心エコー,tilt table test,脳波等には異常を認めなかった.病歴よりFEAを疑い,運動および食物(小麦)負荷誘発試験により血中ヒスタミン濃度の約3倍の上昇を認め,アレルギー素因(金属アレルギー)もあり,FEAと診断した.運動前には小麦を含む食品摂取を避けるよう生活指導を行い,16カ月にわたる観察期間中に失神の再発は認めていない.
This report describes a patient with food-dependent exercise-induced anaphylaxis (FEA). A 25 year-old female has had three episodes of syncopal attack as-sociated with urticaria in the past two years : the attacks have been developed by exercise after taking meals made of wheat flour. She has an allergic predispo-sition to metals. No abnormal findings were observed in the routine work up for syncope including, ECG, 24-hour Holter ECG, UCG, tilt table test, and EEG examina-tions. A provocative test for FEA was performed.It included food (sandwich) intake followed by exercise.
