失神を契機に診断された異型狭心症の1例

Syncope Affording an Opportunity for the Diagnosis of Variant Angina without Chest Pain: Report of a case

　症例は51歳，男性．慢性B型肝炎の治療のため入院中，午前5時頃就寝中に，突然嘔気を自覚し覚醒し，その後失神した．過去に同様の失神を2回認めている．ホルター心電図施行中に再度失神発作を起こした．その記録にて，ST上昇，心室頻拍，房室ブロックを認めた．冠動脈造影上，左右冠動脈には器質的病変は認めなかった．アセチルコリンによる冠動脈攣縮誘発試験により右冠動脈に冠攣縮を認めた．以上より，異型狭心症による失神と診断し，Ca拮抗薬および硝酸薬にて加療を行った．その後，1年を経過するが，失神発作は認めていない．胸痛を伴わない失神発作の原因として，異型狭心症も念頭におく必要がある．

A 51-year-old man had a syncope occurring in theearly morning without chest pain. By means of Holtermonitoring. ST-segment elevation, ventricular tachy-cardia and atrioventricular block were documented justprevious to the patients losing consciousness. Coronaryarteriography showed normal coronary arteriograms,but coronary arterial spasm was induced at segment 3by acetylcholine provocation test. He was diagnosed ashaving variant angina, which was responsible for hissyncope. The patient has been effectively treated withcalcium-blocking agents and nitrates.
