ペースメーカー植え込みを契機として上大静脈血栓・菌血症を発症した1例

Pacemaker Infection with Superior Vena Cava Thrombotic Obstruction, Followed by Bacteremia:A Case Report

　経静脈性心内膜ペースメーカーの合併症の一つとして感染がある，今回我々はペースメーカー植え込み後血栓による上大静脈の閉塞を発症し，5年後植え込み部位の感染から引き続き菌血症となった症例を経験した．ガリウムシンチ・静脈造影などの画像診断により上大静脈内血栓部位の感染が考えられた．内科的に根治的治療は困難なため開心術を施行した．手術では上大静脈には器質化した血栓とペースメーカーリードにvegetation（1cm×1cm）の付着を認め，感染源と考えられた．ペースメーカーリードを摘出し手術を終えたが，術後一時肺炎を合併したものの抗生物質投与後の内科的治療で軽快退院した．術後のガリウムシンチでは縦隔部位の炎症所見は消失していた．

Infection is one of the complications brought about sometimes by pacemaker implantation. This is a case report, showing superior vena cave obstruction after pacemaker implantation, followed by bacteremia after 5 years. Gallium-67 scintigraphy was the most effective method to survey the sorce of the infection. We perfor-med the operation and found vegetation (1 cm×1 cm) attaching to the pacemaker lead and an organized thrombosis in the superior vena cava. The lead and the vegetation were removed. The administration of antibi-otics after the operation led to the patient's complete recovery.
