The effects of combined use of cap topril and diuretic on patients with hypertension in acute aor-tic dissection were reported. Captopril, in a dose of 480 mg daily, was given together with furosemide, in a dose of 160 to 320 mg daily, to two patients with severe hypertension in acute aortic dissection. Sustained control of blood pressure was achieved without side effects. Plasma aldosterone and sodium fell, whereas plasma renin activity and angiotensin I increased. The hemodynamic effects of the two durgs were measured by a Swan-Ganz catheter in one patient. The cardiac output decreased andthe systemic vascular resistance increased. These data indicate reduction of circulating blood volume and stimulation of renin-angiotensin system by furosemide are important factors in hypotensive action of captopril.

Captopril, angiotensin converting enzyme inhi-bitor, has become widely accepted for treatmentof patients with hypertension and it has been shown to be an effective drug in management of mild or moderate hypertension. Now we reported efficacy of combined use of captopril and furosemide in severe hypertension in acute aortic dissection, hy-pertensive crisis.
