全肺CO2再呼吸法によるA—v DCO2の検討

Studies of A-v DCO2 in the total lung rebreathing method

　全肺CO₂再呼吸法によって，混合静脈血P_CO2（Pv_CO2）を非観血的に測定する事は1950年代より多くの研究者により試みられている^1〜4）。また得られたPv_CO2を使って，Fickの原理による心拍出量の非観血的測定がなされ，臨床的応用の可能性が議論されている^5〜8）。さらに，実際のPv_CO2，Pa_CO2と再呼吸によるPA_CO2とを比較して，肺胞と血液の間でガス交換がなくなった時に，正の，A—vD_CO2，A-aD_CO2が観察されており，この現象の解釈として血液のCO₂不平衡説^9,10）と肺毛細血管膜の電気的負荷電によっておこるとする考え^11）とが対立している^12）。しかし，A—vD_CO2を議論する前に全肺CO₂再呼吸法そのものの基準を明確にする必要がある。

Many investigaters suggested that the positive A-vD_CO2 existed in a total lung rebreathing method. They used various initial bag P_CO2, value, and that is considered to influence the value of A-vD_CO2. The auther investigated the relation between the initial P_CO2 value in the bag and A-vD_CO2 in order to find out the CO₂ gradient between alveolus and lung capillary, and to seach the possibility of application of rebreath-ing P_CO2 instead of Pv_CO2.

When the initial P_CO2 value in the bag was between Pv_CO2 and Pv_CO2+9mmHg, the mean A-vD_CO2 was 2.48mmHg and after Haldane cor-rection it was -0.17mmHg. When the initial P_CO2 value in the bag was between Pv_CO2+9mmHg and Pv_CO2+15mmHg, the mean A-vD_CO2 was 5.24mmHg and after Haldane correction, it was 2.14mmHg. When the initial P_CO2 value in the bag was less than Pv_CO2 or more than Pv_CO2+15 mmHg, measured PA_CO2 was not equi-librated before 20 seconds.
