 22 patients with mitral valve disease (MR 11, MS 11) were studied to observe the effects of PGE1 on cardiovascular and respiratory systems during anesthesia with morphine. Hemodynamic parameters were HR, AP, PAP, PCWP, CVP, and CO. After 30 minute equilibrium period these measurements were performed as control. PGE1 was infused intravenously at a rate of 0.1 μg/kg/min for 15 minutes and the same parameters as control were measured. These measurements were completed prior to the start of operation.

 In MR group PGE1 produced a 9.6% decrease in AP, a 27% increase in CI, a 26% increase in SI and a 27% decrease in SVR. In MS group PGE1 produced a 26% decrease in AP, a 10% increase in CI and a 25% decrease in SVR. PGE1 did not produce significant changes in HR, PCWP, PAP, CVP, Pao2, Qs/Qt and A-aDo2 in both groups. PGE1 seems to produce arterial vasodilation and a reduction of impedance to left ventricular outflow. From our study these findings suggest that the action of PGE1 is rather short and may be used as a vasodilator during anesthesia for cardiac patient with high SVR, or low CI without any vigorous change on venous side.
