Myocardial sarcoidosis is a serious and not in-frequently a fatal disease. It is difficult to establish the clinical diagnosis of myocardial sarcoidosis. Electrocardiographic findings are considered to be an important indication of cardiac involvement in its early stage. Therefore, electrocardiograms taken from 963 patients with sarcoidosis of various organs which were collected from Japan Sarcoidosis Committee were analyzed and com-pared with 946 sex-age matched healthy controls.

Abnormalities were found in 217 patients with sarcoidosis (22.1%) which was a higher incidence than in the controls. (p<0.025). Particulary, a higher incidence in woman over 40 years old was recognized. There was a higher incidence of right bundle branch block, premature ventricular con-traction, ectopic atrial rhythm and ST-T changes than in the controls (p<0.05). During the follow up period of up to 16 years (mean observation period 2.4 years), serial electrocardiograms were in 240 patients. In 58 of these, further ab-normalities, i, e., premature atrial contraction, premature atrial contraction, right bundle branch block, A-V block appeared. During the follow up period, electrocardiographic findings showing deterioration was more frequent in the sarcoidosis group than in the controls.
