ペーシングパルスおよびQRS信号電話伝送によるペースメーカー植え込み患者管理の試み

Pacemaker patient follow-up system by transmitting pacemaker and QRS signals

　心臓ペースメーカーの植え込みは，房室ブロックやSick Sinus症候群などに対する確立した治療法として毎年多くの患者に対し行われており，これらのペースメーカー植え込み患者は年々累積してきている。これらの患者の定期的follow-upは重要な問題であり，最近では電話伝送による定期的患者管理システムが脚光をあびてきている^1）〜3）。現在行われている方法は心電図の電話伝送であるが，電話伝送装置が高価なために充分に普及していないのが現状である。

　ペースメーカー患者の定期的管理上必要最小限度の情報としては，1）患者の臨床症状，2）ペーシングレート，3）ペーシングパルスによる心室捕捉，4）感知機能であるが，2），3），4）の情報を得るためであれば必ずしも心電図でなくとも，ペーシングパルスとQRSを伝送させ処理することにより可能である。そこで著者らは，ペーシングパルス信号およびQRS信号電話伝送システムを開発し，臨床応用を試みたので結果を報告する。

A new telephone transmitting system for follow-up of the pacemaker patients was develop-ed. The principles of the system are 1) identi-fication of pacing pulse and QRS, 2) their conver-sion to different sounds, 3) telephone transmission, and 4) their receiving and recording as a phono-cardiogram. Application to 22 paced patients proved that the new system was useful and reliable. The cost of the system is less than one tenth of the EKG transmitting system.
