単心室症における心室特性（容量，機能）の研究およびその外科的治療法における意義

Ventricular Functional Characteristics and Their Surgical Implications in Single Ventricle

　単心室症は先天性心奇形の中で最も複雑なものの1つでそれに対する外科的治療法の開発も遅れている分野である。しかし本症は我が国に比較的多いように思われ大阪大学だけでも50例以上を経験している。また最近本症に対する根治手術（心室2分割術—心室中隔作成術，septation）^1〜11），機能的根治手術（右房—肺動脈バイパス術）^12）の報告も増加して来ておりこの分野における我が国の貢献度は高く評価されるべきと考える。

　外科的根治術の可能性が高まるにつれ単心室症の有する心室容積機能の特異性を把握することが重要となって来たが未だ単心室症における心室機能の研究は皆無である。そこで著者らは大阪大学で心血管造影検査（ACG）を受け，単心室症と診断された34例を対象としてその心室容積心機能を検討したので報告する。更にこのうち5例に一期的根治手術が行われて2例に成功を収めている^3,11,14）のでこれらの手術例における心機能をrestrospectiveに検討する。

Anatomical characteristics of single ventricle (SV) have been well described and this anomaly has recently been becoming an attractive challenging subject for corrective surgery. However, the functional characteristics of SV have not yet been reported. Analysis of biplane ventricular angiograms taken in the usual diagnostic studies in 34 patients (pts) with an angiocardiographically or autopsy confirmed diagnosis of SV formed the basis of this report.
