体外循環後呼吸不全に関与する液性因子—血漿中に存在する肺細胞傷害物質のin vitro証明法について

　体外循環後にみられる肺細胞傷害をきたす液性因子を追求した。初代肺培養細胞を標的細胞として被検物質とともに，incubateし傷害肺細胞から浮遊液中に放出されるLDH量を測定することで，肺細胞傷害作用の定量化を行うことに成功した。

　標的細胞としては，初代培養家兎肺細胞を利用できることがわかった。

　正常人血漿中にも，分子量14〜16万dalton，α₁—glo-bulin分画に属する肺細胞傷害物質がある。

　開心術を受けた患者9人について血漿中の肺細胞傷害物質の変動をみると，体外循環中ではむしろ低値を示し循環後1日目になっても循環前値には達しなかった。

　肺細胞を傷害するには，基底膜破壊その他の原因で肺血管透過性の亢進が起こり，血中の肺細胞傷害物質が血管外に濾出することが必要である。肺細胞の傷害が著明な場合，体外循環後呼吸不全発症の一因になると推論した。

Cultured human lung cells were incubated with various fractions of the serum components, and the lactate dehydrogenase (LDH) released from the affected lung cells was measured as an index of the impairment.
