開心術後の血清catecholamines（epinephrine，dopamineとnorepinephrine）よりみた血行動態

Serum Catecholamines (Epinephrine, Dopamine and Norepinephrine) Following Open Heart Surgery

　心機能を評価する指標が等しくても，比較する心の被刺激量を知らずに機能が等しいとはいい難いであろう。とくに予備力が等しいともいえないであろう。すなわち，ある一定の心機能を保つために，刺激を受けている心と，全く刺激を受けていない心がありうるからである。たとえばcontractilityが低下した心筋でも，catecholarnine levelが高ければcontractilityは増強し，catecholamineに刺激されていない健常な心筋のcontractilityに等しく，または近づくことはありうるであろう。

　したがって，開心術後でも，最大の刺激により心機能が保持されている患者と，ほとんど刺激されることなく心機能が保持されている患者があると考えられ，この両者の鑑別も開心術後患者の管理にはきわめて重要と思われる。

　An effort was made to analyze factors of administrating isoproterenol following open heart surgery in 36 patients with tetralogy of Fallot. Then, in other 41 patients with various kinds of heart diseases, serum catecholamines, including epinephrine, dopamine and norepinephrine, and cardiac index were determined at 2,4 and 6 hours after open heart surgery.

　Statistically significant correlation was found between extracorporeal circulation time and score in which larger points were given in time belt closer to operation.
