　われわれは最近，うっ血性心不全を反復し心電図上1度房室ブロック，完全左脚ブロックを示し，ときに一過性に左脚ブロックが消失して異常左軸偏位のみを現わし，最終的に心房細動および心室頻拍の発生後に死亡したうっ血型心筋症と思われる1症例を経験し，その刺激伝導系をLevの方法^1）により詳細に検索を行った。生前の心電図およびHis bundle electrogram （以下HBEと略す）所見と病理組織所見とを対比し，両者の間の密接な関連性および問題点について若干の考察を加えたので報告する。

72 year-old man was first admitted to the Kyoto City Hospital on Feb. 13, 1974, because of shortness of breath and nocturnal dyspnea. The diagnosis on congestive heart failure was made, and the electrocardiogram revealed QS pattern in V₁ through V₄ resembling an old antercseptal myocardial infarction, first degree AV block and complete left bundle branch block (CLBBB). The His bundle electrogram recorded on March 6, 1974 showed the prolongation of PA and HV intervals in normal sinus rhythm.
