いわゆるSinus Node Re-entryについて

So-called Sinus Node Re-entry

　日常の心電図で心房性早期収縮が現われると，その早期収縮のP波から次に現われるP波までの時間（return cycle）が早期収縮の前の基本洞周期（dominant cycle）より延長する現象はよく認められるが，短縮する例は比較的稀である^1,2）。

　しかし，これらの現象もヒス束心電図（HBE），心房早期電気刺激（PAD）の臨床への導入により詳細に観察されるようになり^3〜8），Narula^4），Weisfogelら^5）は異常に短縮したreturn cycleを示すものの中にはsinus nodere-entry （SNR），またはsinus node re-entrant tachy-cardiaで説明しうる現象が存在することを指摘している。

In 13 patients with normal sinus rhythms, sinus node re-entrant phenomenon was observed follow-ing induced atrial premature deporalization (PAD), and 2 patients with LGL syndrome revealed sinus node re-entrant tachycardia. Right heart cathe-terization was performed in all the patients. A bipolar electrode was used for His bundle record-ing, and another quadripolar electrode was positioned in the high right atrium for stimuration and recording high right atrial electrical activity.
