HV時間短縮を示すPQ短縮，幅せまいQRS症例について

Short PQ, narrow QRS, and short HV interval

　心電図でPQ短縮と幅のせまいQRSを現わす症例ではPQが正常のものに比し発作性頻拍を発生する率が高く，いわゆるLown-Ganong-Levine症候群として注目されている^1）。

　近年カテーテル電極を用いてHis束心電図が容易に記録できるようになり^2），PQ短縮と幅のせまいQRSを示す症例でもHis束心電図的研究が行われるようになった^3）−9）。これらのうち共通してみられるものはAH時間が比較的短いことで^3）−9），われわれもAH時間については既に報告した^8）9）。一方Mandelら^4）は彼らが報告した3例全例においてHV時間の短縮を報告している。われわれも自験例19例中7例においてHV時間の短縮の傾向をみとめているので，今回はHV時間短縮例のみについて検討を加え報告する。

Nineteen patients with a short PQ interval (<0. 12 second) and narrow QRS (<0. 10 second) were studied by means of His bundle recordings, artificial atrial pacing, and programmed premature atrial depolarization. In some patients vectorcardiograms (Frank system) were recorded simultaneously with His bundle recording during sinus rhythm and when the QRS complexes of the electrocardiograms changed in shape by atrial pacing.

In all patients the AH interval was less than or at the lower limits of normal value during sinus rhythm.
