拡大気管支造影法によるSmall airway dynamicsの研究（その1）—定量的解析と各種麻酔剤について

Study of small airway dynamics using magnification bronchography: No. 1-Establishment of quantitative analysis and the effects of various anesthetics on small airway

　Small airway diseaseの早期検出の手掛りとしてclosing volumeをはじめ種々の生理学的検索法が検討されている^1,2）。同時に解剖学的形態学的立場からも気管支の性状が検討されている^3〜5）。

　1968年Nadelらによって開発された造影剤tantalum powderの使用と微小焦点X線管球併用による拡大気管支造影法はin vivoにおいて細気管支領域を鮮明に解像することを可能にした^7,8）。Tantalum powderは他の造影剤と異り幾つかの利点を持つが，その最大の特長は肺機能を障害しない点である^6,7,9）。この観点からtantalum bronchographyによるsmall airway dynamicsの形態学的検討が広く行われつつある^4,5,10）。しかしながら，塗布されたtantalum powderの気管支に及ぼす影響—特にtantalumの気管支に及ぼす経時的影響についていまだ検討されておらず，定量的解析を行う上で問題となる点である。またbronchographyを行うにあたって麻酔が必要であり，その際の麻酔剤の影響も無視することはできない。

Changes in bronchial dimension have usually been estimated from indirect measurements of physiological parameters. Microfocal magnification radiography using insufflated tantalum powder have provided an opportunity to identify the anatomical site of airway dimensional change produced by various agents, and to demonstrate the change in bronchial muscle tone by "direct" visualization. However, before diseased small airways can be studied, dynamic characteristics of normal experimental conditions using tantalum must be assessed.
