B型WPW症候群の1例—心室刺激による頻拍旋回路に関する考察

　多くの発作性上室性頻拍は，房室結節内のreentryによって生じると考えられている^1,2）。一方，WPW症候群でみられる多くの発作性上室性頻拍では，正常房室伝導路を正伝導し心室へ達し，Kent束を介して心房へ逆伝導する旋回路の関与が示唆されており^2〜8），最近の本頻拍に対するKent束の切断術^7〜9）はこの機序を立証したといえる。Kent束が頻拍旋回路の一部に含まれているかどうかを鑑別することは，この手術の適応を決定する上でその重要性を増したが，なお困難な場合が多い^2,10）。

　われわれは，atropine投与後にのみ頻拍が誘発されたB型WPW症候群の1例に，薬物投与前後で高頻度心室刺激を試み，正常房室伝導路を正伝導しKent束を逆伝導する頻拍であると診断した。そこで，この機序による頻拍と房室結節のreentyによる頻拍を鑑別するさいのatropine投与前後での心室刺激の有用性と，頻拍の維持に対する迷走神経の関与について考察したので報告する。

Three years prior to admission, a 17-year-old boy noticed episodes of palpitation during exercise and was first discovered to have Wolff-Parkinson-White (WPW) syndrome. The patient was admitted for electrophysiologic evaluation. Three electrode catheters were introduced for recording His bundle electrogram (HBE), high right atrial potential (HRA) and low lateral right atrial potential (LLRA), and also for pacing right atrium and ventricle.
