先天性心奇形，とくに心房中隔欠損に合併した僧帽弁逸脱症候群

Association of mitral valve prolapse and congenital heart disease, particularly atrial septal defect

　心房中隔欠損に合併した僧帽弁逸脱症候群の存在が，最近とみに注目されるようになってきた^1〜4）。本症候群はmid-systolic click-late systolic murmur syndromeという名で一括されるごとく，特異な聴診，心音図所見により印象づけられてきた。その後，本症候群の心エコー図所見がDillonら^5）により報告されて以来，心エコー図や超音波心臓断層図により本症候群の成因を解明しようとする努力もなされている。

　今回の研究の目的は，先天性心奇形の中でとくに心房中隔欠損（atrial septal defect：　ASD）とEbstein奇形に本症候群が高率に合併する事実を報告し，その診断において心音図，心エコー図，心電図がどのような役割を果すかを明らかにすることにある。

A total of 114 patients with congenital heart disease was studied by left ventriculography, echocardiography, phonocardiography, and electrocardiography. Angiographically documented mitral valve prolapse was noted in 8 of 22 children with atrial septal defect, 12 of 19 adults with atrial septal defect and 2 of 4 patients with Ebstein's anomaly associated with atrial septal defect.
