A case of strictly posterior myocardial infarction with several unusual complications was presented. Sixty-four years old man had experienced an episode of long-standing chest pain with electro-cardiographic evidences of strictly posterior and lateral wall myocardial infarctions. On admission to the hospital, his electrocardiogram showed the findings of left posterior hemiblock. One week after the attack, a friction rub appeared, and the diagnosis of post-myocardial infarction syn-drome was made based on serum immunological studies. The electrocardiographic changes and the friction rub disappeared within one month period.
