 NEN(neuroendocrine neoplasm)is tumors differentiating into endocrine cells of the gastrointestinal tract mucosa, including well-differentiated NET(neuroendocrine tumor), poorly differentiated NEC(neuroendocrine carcinoma), and MiNEN(mixed neuroendocrine-non-neuroendocrine neoplasm). NEC/MiNEN are not derived from NET. Classifying gastric NET by hypergastrinemia, that is, ECL(enterochromaffin-like)-cell NET derived from histamine-producing ECL cells proliferating in the fundic glands due to hypergastrinemia and other NET is deemed crucial since Rindi et al. report(1993). Hypergastrinemic NET associated with atrophic gastritis are classified as type 1 ECL-cell NET, NET associated with Zollinger-Ellison syndrome as type 2 ECL-cell NET, and sporadic NET not associated with these conditions as type 3 NET. NET in the setting of parietal cell dysfunction are now classified as type 4 ECL-cell NET. In this section, NET related to the long-term administration of proton pump inhibitors are stratified into type 5 ECL-cell NET. In the clinicopathological setting, including treatment strategies, it is pivotal to subtype gastric NEN according to the underlying disease and histopathology of the background mucosa.
