要旨●近年の飛躍的なテクノロジーの進化に伴い,内視鏡医が取得する画像の質・データ量は格段に向上している.しかし,求められる診断技術のハードルも同時に向上しており,高精度の内視鏡診断が一部の熟練医に限られているのも事実である.このような先端機器と医療技術のギャップを解決することを目的として,人工知能(AI)を用いたコンピュータ診断支援システムの研究開発が注目を浴びており,本邦でも一部製品の市販が始まっている.本稿では,大腸内視鏡AIの研究状況を概観するとともに,薬事承認の状況についても紹介し,今後の診療現場に内視鏡AIがどのような影響を与えうるのかを考察する.
Along with the dramatic advances in imaging technologie in recent years, the quality and quantity of endoscopic images have increased significantly, which raise the hurdle to a high-performance diagnosis. Thus, only a limited number of expert endoscopists can achieve such a highly accurate diagnosis. To overcome this issue and allow any endoscopist to provide a high-quality diagnosis regardless of their skills or environment, CAD(computer-aided diagnosis)using AI(artificial intelligence)is becoming increasingly popular. A recent rapid AI development is realizing a CAD implementation into real clinical endoscopy. This review overviews the current research situation on AI for endoscopy in the colon. Simultaneously, we mention barriers that are hindering AI clinical implementation for endoscopy and regulatory approval importance, which will facilitate the understanding of the current status of AI-assisted colonoscopy.
