咽頭表在癌に対する内視鏡診断—範囲診断・深達度診断

Endoscopic Observation of the Laryngopharynx

要旨●本稿では，咽頭表在癌の範囲診断と深達度診断について，実際の症例を提示しながら解説する．咽喉頭は，反射があることや解剖学的構造が複雑であることから，観察・診断が難しい部位である．しかし，意識下鎮静での発声やバルサマウス®などを用いたうえで，NBIなどの画像強調内視鏡観察と拡大観察を行うことによって，病変の範囲診断や深達度診断が可能である．また，治療時には全身麻酔下で喉頭展開し，ヨード染色により範囲診断を確認することも重要である．

　Endoscopic observation of the laryngopharynx is technically challenging due to the gag reflex and the complicated anatomical structure. However, it is possible to determine the size and depth of tumoral invasion using image-enhanced endoscopic techniques such as narrow band imaging and carefully magnified visualization after vocalization with conscious sedation. Further visualization of the larynx under general anesthesia may be important to assess the extent of disease using iodine staining. In this study, we describe endoscopic diagnosis of superficial pharyngeal cancer in three cases treated at our hospital.
