アカラシアおよび関連疾患の診断—内視鏡の立場から

Diagnostics of Benign Esophageal Functional Disorders:Endoscopic Prospect

要旨●食道アカラシアおよびその類縁疾患の診断および治療は，近年新たな低侵襲治療の内視鏡的食道筋層切開術（POEM）やhigh resolution manometryが登場したことによって飛躍的に進歩した．運動機能の異常である本疾患群は，形態学のみでは診断困難な場合も多く，内視鏡の典型像を知る以外にも，食道X線検査や食道内圧測定などの検査所見から総合的に病態を把握することが重要である．本稿では，食道アカラシアや遠位食道痙攣，jackhammer食道などの食道運動機能障害の内視鏡所見について概説する．

　Diagnostic and therapeutic strategies for esophageal achalasia and related diseases such as distal esophageal spasm and jackhammer esophagus have radically evolved owing to the introduction of POEM and high-resolution manometry. Comprehensive studies via endoscopy, barium esophagography, and manometry are mandatory to understand the pathology and select appropriate treatment approach to these functional disorders. In this article, typical endoscopic findings of esophageal achalasia and related diseases are reviewed.
