潰瘍性大腸炎に対するサーベイランス—SCENIC international consensus statementの概要と問題点

Development of SCENIC International Consensus Statements for Colonoscopic Surveillance in Patients with Ulcerative Colitis

要旨●2015年に欧米の研究者を主要メンバーとした潰瘍性大腸炎（UC）の大腸癌サーベイランス法に関する国際会議が開催され，合議の結果がSCENIC国際ステートメントとして発表された．このステートメントでは，臨床データに基づいたUCのサーベイランス法が検討され，色素内視鏡検査による観察と境界明瞭な病変に対する内視鏡的切除が推奨されている．本ステートメントの発表以降，欧米では画像強調内視鏡検査を用いたUCの腫瘍性病変の診断法に関する臨床研究と内視鏡治療に関する議論が活発となった．すなわち，SCENIC国際ステートメントを契機として，欧米では潰瘍性大腸炎のサーベイランス内視鏡に対する考え方が大きく変化したと言えよう．

　SCENIC-ICS（Surveillance for Colorectal Endoscopic Neoplasia Detection and Management in Inflammatory Bowel Disease Patients International Consensus Statement）refers to statements regarding the procedures of colonoscopic surveillance and descriptions of UCAN（ulcerative colitis-associated neoplasia）. SCENIC-ICS recommends chromoendoscopy for the detection of UCAN and endoscopic resection for the management of well-circumscribed lesions.

　Following the complete removal of endoscopically resectable UCAN, continuing surveillance colonoscopy is suggested rather than colectomy. Since the publication of SCENIC-ICS, there have been rigorous discussions regarding the endoscopic diagnosis and management of UCAN in Western countries. This suggests that SCENIC-ICS has provided an initial step for the establishment of up-to-date, standardized surveillance colonoscopy for UCAN in the era of advancement of endoscopic instrumentation.
