表在型咽頭癌が形態変化を来す要因

Morphological Changes in Superficial Pharyngeal Cancer

要旨●表在型中・下咽頭癌が多く発見されるようになる一方で，その生物学的特徴について明らかにすることが求められている．筆者らのデータでは，自然経過で多くの症例が緩徐な発育を来し，全体として年平均4mmのサイズの増大が認められた．しかし，急速に増大した症例もあることから，少なくとも半年ごとのフォローが必要である．化学療法における反応性に関しては，奏効率が61%，病勢コントロール率は97%であり，良好な成績が得られた．生検による変化については，限られた症例であるが形態変化を来す場合が経験される．したがって，形態変化を来した部位の詳細な観察とともに，生検部位の確認は必要である．こうした動態を踏まえた表在型咽頭癌に対する適切な治療方法の選択が望まれる．

　The incidence of superficial pharyngeal cancer has been rising recently, and clarifying the biological features of the disease is imperative. When we examined tumors without treatment intervention, we found that the tumor size increases at an average of 4mm annually. However, in a few cases, we observed a rapid increase in tumor size. Therefore, the tumors should be examined at least semi-annually. In patients who underwent chemotherapy, the response and disease control rates were 61% and 97%, respectively. In terms of morphological changes after biopsy, protrusions were observed at the site of biopsy in a few cases. While considering treatment options for superficial pharyngeal cancer, the appropriate option should be selected on the basis of these data.
