要旨　レーザー光を光源とした内視鏡システムによる狭帯域光観察であるBLI（blue laser imaging）は大腸腫瘍の診断に有用であり，拡大観察においては明瞭なvascular patternおよびsurface patternの描出が可能である．LST（laterally spreading tumor）の診断においては，SM深部浸潤の頻度が高いとされるLST granular typeのnodular mixed typeやLST non-granular typeのpseudo depressed typeにおいて良好な表面構造の描出により精密な診断が可能となる例も少なくはない．本稿では当科での経験を踏まえて大腸LSTのBLI診断について詳述する．

　Blue laser imaging（BLI）in an endoscope system with a semiconductor laser light source allows for narrow-band observation. The BLI light is useful for acquiring mucosal surface information such as vascular and surface patterns. In laterally spreading tumors with submucosal invasion, such as flat elevated type and pseudo depressive type, BLI can provide the description of clear surface patterns. In this chapter, we introduce the efficacy of BLI for the diagnosis of laterally spreading tumors.
